Phyllis Jean Choate Simmons, age 80 and a lifetime resident of Hickman, Ky., passed away Sat., May 17, 2025, at Clinton Place in Clinton, Ky.

Phyllis was born in Hickman,  on Aug. 7, 1944, to the late Phillip and Bethel Edwards Choate. 

She was a member of the Sacred Heart Catholic Church in Hickman,  and graduated from the University of Tennessee at Martin with a degree in Home Economics. She retired from her many years of service with the University of Kentucky Cooperative Agency as the Home Economics Extension Agent for Hickman County, Ky. She was a member of the Daughters of the American Revolution. She was an avid genealogist and a long-time serving member of the Hickman Public Library Board.
She is survived by her two sons, Charles Edward (Chip) Simmons Jr. (Stephanie) of Lexington, Ky., and Daniel Choate Simmons, Poplar Bluff, Mo., and her twin granddaughters, Molly and Maggie Simmons both of Lexington, Ky., one brother, Al Choate (Betsy), Madisonville, Ky.; one sister-in-law Karen, Muskegon, Mich.
In addition to her parents, she is preceded in death by her husband, the love of her life, Charles (Charlie) Edward Simmons, who passed away in 1984; and one brother, Phil Choate.
A Funeral Mass will be held for Phyllis Simmons at 12 noon on Friday, May 23, 2025, at the Sacred Heart Catholic Church in Hickman, Ky. Burial will follow in the Hickman City Cemetery. Visitation will be held at the Church beginning at 10 a.m. Friday, May 23.
